Compare the facts that are published
The Louisville location publishes weekday office hours of 8:30 AM to 4:00 PM. Radcliff publishes 9:00 AM to 4:00 PM. The official contact page lists (502) 995-4004 for the practice and says phones are answered 24/7. Confirm the appointment location and time when scheduling.
The official location pages describe their surrounding areas, but travel time depends on your origin and current conditions. Check a route from your own address; do not treat a neighborhood mention as evidence that one clinic is always closer.
Ask about the specific appointment
The practice-wide treatment directory lists sixteen services. It does not publish a current appointment calendar, insurance contract by office or a procedure-specific availability table. If you need a particular type of visit, ask scheduling which office can provide it on the dates available to you.
A consultation may be the first step before the clinician determines whether a listed procedure is appropriate. Location choice and treatment choice are separate decisions; a directory link is not a promise that a procedure will be performed at a particular visit.
Use one comparison checklist
Record each address, the proposed appointment time, your insurer's referral or authorization requirement, and any preparation instructions. The official location pages say many patients schedule directly but some plans require a primary-care referral.
If an older review or third-party provider listing uses a different address, the official current contact and location pages are the right starting point for confirming the destination with staff.
Questions to ask
- Which office is available for the type of appointment I need?
- Is the appointment at Louisville Suite A or the Radcliff address?
- Are there office-specific preparation or check-in instructions?
- Does my plan require a referral or authorization for this visit?
