Use the Chamberlain Lane address from the official Louisville page when planning the trip. The suite letter matters: the practice lists Suite A. If your appointment materials show another location, ask the scheduling team which office is assigned before setting out.
The official page describes the office as serving Louisville's East End. It names nearby communities as context, but does not publish a drive-time guarantee. Check your own route and allow for traffic, parking and check-in time based on the appointment instructions you receive.
