Verify the appointment first

The practice’s location pages say many patients schedule directly and some insurance plans require a primary-care referral. Confirm the requirement for your plan, not just the general practice policy. Ask whether the clinician and intended facility are in network and whether the consultation itself is covered.

Before a procedure, use the proposed service name or billing code from the office to ask the plan about prior authorization, site-of-service rules, and patient responsibility. Authorization is a process check; it should not be treated as a guarantee of final payment.

Record the answer

Keep the plan representative’s name or reference number, date, and exact question asked. If multiple billers are involved, check each one. If the care plan changes, repeat the coverage check for the new service or setting rather than relying on the earlier answer.

Medicare’s own outpatient guidance points patients to the clinician for cost questions and explains that facility type and other coverage can affect what someone owes.

Distinguish referral from authorization

A referral is a route into specialist care under some plans. Prior authorization is a plan review for a proposed service. They are separate questions, and the answer for an initial evaluation may differ from the answer for a later procedure. Ask the practice and insurer which step applies, who submits paperwork and how you will know the decision.
