Kentuckiana Pain Specialists Reviews — Epidural versus facet injection

An epidural injection and a facet procedure address different suspected pain sources. A referral that only says back or neck pain is not enough to choose between them.

Decision guide · Updated October 6, 2026

Compare the suspected source

An epidural approach places medicine around spinal nerves and may be considered for a nerve-root pain pattern. Facet procedures focus on small spinal joints or their nerve supply. Ask which symptoms, examination findings, and imaging support the chosen target.

The proposed site is a clue to the working diagnosis. If your symptoms have shifted since the referral was written, tell the clinician before the procedure is finalized.

Compare the purpose

An epidural injection may be proposed for symptom relief. A facet medial branch block can be a short diagnostic test before another procedure; a joint injection can have a different purpose. Confirm the exact medicine and whether the outcome is diagnostic, therapeutic, or both.

A pain drawing that shows whether symptoms radiate into an arm or leg can help frame the visit, but it cannot diagnose the source alone. Report numbness, weakness, fever, or other changes separately so the clinician can decide what needs evaluation first.

Compare the report-back plan

Identify the daily activity that provokes your usual pain and record change after the procedure. A short anesthetic window and a longer steroid response are not interpreted the same way. Ask when the team wants that information.

Ask whether the next step after a negative injection is a new diagnostic assessment or simply a repeat injection. A failed test at one target may be useful information; it should not automatically be treated as proof that all injections are futile.

Questions to ask

  • Do my symptoms fit a nerve root or facet joint source?
  • Is the proposed procedure diagnostic or therapeutic?
  • What result would make us change course?

Treatment

Understanding epidural injection options

The practice separately lists epidural injections and epidural steroid injections. The useful question is what medicine, route, and target the clinician proposes for a specific diagnosis.

Understanding diagnostic facet joint blocks

A diagnostic facet block asks whether nerves serving a facet joint carry the pain being investigated. Its value lies in a well-defined test and careful interpretation of the short response.
