Confirm the exact site
Ask whether the service is performed at the Louisville office, Radcliff office, a hospital or an outpatient facility. A practice-wide service list does not prove that each service is offered at every address. Request the written address, arrival time and parking or check-in instructions for the scheduled appointment.
Understand the team and equipment
Ask who performs the procedure, whether imaging guidance is planned and who provides sedation if it is used. The role of imaging or sedation depends on the procedure and your circumstances. If an implant is involved, confirm which team handles programming and post-implant questions.
Plan the trip home
Ask whether you will need a driver, what activity limits the treating team recommends and whom to call if you are delayed or become ill. Do not rely on a generic online recovery schedule for personal instructions.
A consultation may be scheduled in an office even if a procedure is later done at a facility with a different check-in desk and billing. Ask for the exact name of that facility and whether separate pre-procedure paperwork is required. If a treatment needs repeated visits, confirm whether each visit uses the same location and transportation plan.
